Forms

Build ops forms, approval routing, and CSV export.

Studio → Forms — *"Ops collection with optional manager approval — assign from the detail page."* Creating requires Edit content (content:write).

Create a form

  1. Choose New form.
  2. Enter a Title (for example *Uniform request*) and an optional Category (for example *ops*).
  3. Choose Create draft. New forms start with approval required and a sample Notes field.

Add fields

On the form page, use Add field with a label (for example *What happened?*) and a type:

Fields can be marked required, and conditional fields show a *when {field}={value}* badge. Publishing requires at least one field — otherwise Add at least one field before publishing.

Approval routing

The form header shows approval on or approval off:

  • Approval on (the default): submissions land in the manager Approvals queue with status submitted. Managers Approve (completes the learner's task), Reject, or request changes — rejected forms reopen for the learner with the reviewer note.
  • Approval off: submissions are approved instantly and the task completes on submit.

The approval setting is fixed when the form is created and shown as a read-only badge afterward — recreate the form if you need the other mode.

Publish, assign, export

Choose Publish, then Assign by role and location. Learners fill the form from Tasks (or inside a course when embedded as a form module/block). Export CSV downloads all submissions — this requires the View reports permission.
